Please make sure you let me
know if you are coming and the approximate time. Times at locations could
vary.
If you have spines that need matched up, please make sure you include one
that has been previously engraved.
Forms need to be filled out ahead of time and money included.
There are pictures of album covers, instructions & the order form, which
can be printed out here on this site.

Our order
form is available online. Simply go to the order form page and print the
page while viewing in your browser. Complete the form and mail it to us as
instructed above.
If, because of your printer defaults, the form fails to print correctly, you
can download the form in MS Word or Acrobat PDF formats.
